The Process


Typically, our incident investigations are structured as follows:

  1. Confirm charter
  2. Select and train investigation team
  3. Gather information
  4. Prepare Sequence of Events (SOE) table
  5. Develop Events and Causal Factors (E&CF) chart
  6. Analyse Causal Factors to find Root Causes
  7. Develop Recommendations to correct Root Causes
  8. Validate SOE, E&CF, RCA and Recommendations and develop draft report
  9. Finalise report
